using Sean Martin's ideas on a vac table ,I re-designed it a bit and built one 28x48 in and I am showing the construction and the testing that follows
It works fine

Stan,

It looks like your offset values are off. This could be due to your blade selection. The Irwin blades when installed right up against the stops will yield the exact same result every time with very sharp clean corners.
